Enrollment

 

In order to enroll your child in Chrisalis Home Academy, there is paperwork that is necessary and needs to be completed prior to the beginning of care. We will not assume care of your child(ren) until all paperwork, fees and supplies are received. All information will be kept confidential and is for our records only.

 

PAPERWORK

The following paperwork (including; but not limited to) must be filled out and returned to us by the first day of care.  Paperwork is found in the handbook (link is below).

 

  • Enrollment Record

  • Child Introduction Form

  • Medical Record Form

  • Emergency Medical Consent Form

  • Authorization to Administer Medication

  • Permission to Photograph

  • Emergency Contracts / Permission to Drop off and Pick up

  • Copy of Immunization Forms

Tuition/fees and Payment Schedule

 

Our weekly tuition is $199.00 for fulltime children under age 2.

Our weekly tuition is $185.00 for fulltime children over 2yrs of age until school age.

Military Active/ Reserve Active Duty/Veteran receive a 10% discount off tuition.

 

Field Trips/Activity fees range from free to $20.00 per child depending on the activity. (It is our goal to add no additional cost to parents and when doing so we will try our best to get the lowest cost possible; however we want to provide opportunities for your child / children to experience a variety of activities.) Parents will always receive 30 days notice in advance.

 

All payments are due the Friday prior to child care or that Monday morning during drop off – no exceptions.  Receipts will be given for each payment at time of payment.  If your child misses a day for any reason you are still responsible for payment and nonpayment can result in your child placement being terminated immediately.  We will accept cash, check or credit card. Credit card payments carries a $5.00 processing fee.  Return check carries a $35.00 fee and after the 1st return check, checks will no longer be accepted.

 

Please Note:  This daycare will be closed two weeks out of the year to allow for owner’s vacation, with a minimum of 30 days notice to parents, during this time no tuition will be charged.

 

OVERTIME RATES & LATE FEES

A late pickup charge of $2.00 per minute will start at 6:16pm or 15 minutes after you’re contracted pickup time.

 

All past due tuition will be assessed a $10.00 late payment fee each day at midnight that it is late including weekends. Enrollment will be terminated if an account becomes one week past due and the collection process will begin. Parents will pay all costs associated with the collections of any unpaid debt.

 

TAX STATEMENTS (Tax ID#46-4626867)

We will provide you with your tax statement no later than the 3rd week in January of the following year.
